JOB SUMMARYĀ
The registered nurse plans,Ā organizesĀ and directs hospice care and is experienced in nursing, with emphasis onāÆĀ
community health education/experience.āÆāÆThe professional nurseĀ builds fromĀ the resources of the community to plan and direct services to meet the needs ofĀ individualĀ and families within their homes and communities.Ā
DUTIES & RESPONSIBILITIESāÆĀ
Patient CareĀ
1. CompletesĀ an initial,Ā comprehensiveĀ and ongoingĀ assessmentsĀ ofĀ patientĀ and family toĀ determineĀ hospice needs.āÆāÆProvides a complete physical assessment and history of current andĀ previousĀ illness(es).Ā
2. Provides professional nursing care byĀ utilizingĀ all elements ofĀ nursingĀ process.āÆĀ
3. Assesses and evaluates patientās status by:Ā
a. Writing andĀ initiatingĀ plan of careĀ
b. Regularly re-evaluating patient and family/caregiver needsāÆĀ
c.Ā ParticipatingĀ in revising the plan of care as necessaryĀ
4.Ā UsesĀ health assessment data toĀ determineĀ nursing diagnosis.Ā
5. Develops a care plan thatĀ establishesĀ goals, based on nursing diagnosis and incorporates palliative nursingĀ
actions.āÆāÆIncludes the patient and the family in the planning process.Ā
6. InitiatesĀ appropriate preventiveĀ and rehabilitative nursing procedures.āÆāÆAdministers medications andĀ
treatmentsĀ as prescribed byĀ the physician in the physicianās plan of care.Ā
7. Counsels the patient and family in meeting nursing and related needs.Ā
8. Provides health care instructions to the patient asĀ appropriate perĀ assessment and plan.Ā
9.Ā AssistsĀ the patient with the activities of daily living andĀ facilitatesĀ the patientās efforts toward self-sufļ¬ciencyĀ
and optional comfort care.Ā
10. Acts as Case Manager andĀ assumesĀ responsibility to coordinate patient care for assigned caseload.Ā
CommunicationĀ
1. Completes,Ā maintainsĀ and submitsĀ accurateĀ and relevant clinical notesĀ regardingĀ patientās condition and careĀ
given.āÆāÆRecordsĀ pain/symptom management changes/outcomes asĀ appropriate.Ā
2. Communicates with the physicianĀ regardingĀ the patientās needs and reports changes inĀ the patientās condition;Ā
obtains/receives physiciansā orders asĀ required.Ā
3. Communicates with community health relatedĀ personsĀ to coordinate the care plan.Ā
4.Ā TeachesĀ the patient and family/caregiver self-care techniques asĀ appropriate.āÆāÆProvides medication, diet and other instructions as ordered by the physician and recognizes andĀ utilizesĀ opportunities for health counselingĀ
with patients and families/caregivers.āÆāÆWorks in concert with the interdisciplinary group.Ā
5. Provides andĀ maintainsĀ a safe environment for the patient.Ā
6.Ā AssistsĀ the patient and family/caregiver and other team members in providing continuity of care.Ā
7. Works in cooperation with the family/caregiver and hospice Interdisciplinary Group Members to meet theĀ
emotional needs of the patient and family/caregiver.āÆĀ
8. Attends interdisciplinary group meetings.Ā
Additional DutiesĀ
1. Participates in on-call duties asĀ deļ¬nedĀ by the on-call policy.Ā
2. Ensures that arrangements for equipment and other necessary items and services are available.Ā
3. Supervises ancillary personnel and delegates responsibilities whenĀ required.Ā
4. Assumes responsibility for personal growth and development andĀ maintainsĀ and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-serviceĀ
classes.Ā
5. Fulļ¬lls the obligation of requested and/or accepted case assignments.Ā
6. ActivelyĀ participatesĀ in qualityĀ assessmentĀ performance improvement teams and activities.Ā

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
1. Graduate of National League for Nursing accredited school of nursing.Ā
2. Minimum of two years of experience, at least one of which isĀ in the area ofĀ public health, home care, or hospiceĀ nursingĀ is preferred.Ā
3. Management experienceĀ notĀ required.āÆāÆResponsible for supervising hospice aides.Ā
4. Registered nurse with current licensure to practice professional nursing in the state.Ā
5. Demonstrates excellent observation, problem solving, verbal and written communications; nursing skills perĀ
competency checklist.Ā
6. ShowsĀ abilityĀ to organize and prioritize workload independently.Ā
7. Prolonged orĀ considerableĀ walking or standing.āÆāÆAble to lift, position, and/or transfer patients.āÆāÆAble to lift supplies and equipment.āÆāÆConsiderable reaching, stooping, bending, kneeling, and/or crouching.āÆāÆVisual acuityĀ
and hearing to perform required nursing skills.Ā
8. Maintains a current CPR certiļ¬cation.Ā
9. The ability to drive and/or have access to transportation forĀ agency relatedĀ travel (e.g.Ā community partners,Ā
patients, etc.) is an essential function of the job.āÆāÆMust have andĀ maintainĀ a valid driverās license,Ā maintainĀ automobile insuranceĀ coverageĀ and have access to an automobile.Ā
10. Self-directed and able to work with minimal supervision.Ā
āÆPHYSICAL REQUIREMENTSĀ
