Job Summary

We are seeking a dynamic and compassionate Lead Care Manager to oversee and coordinate comprehensive Enhanced Care Management (ECM) services under CalAIM for Health Advocacy Team Support (HATS). This pivotal role involves conducting comprehensive assessments, developing individualized care plans, and facilitating resource coordination across Monterey, Santa Cruz, San Benito, and Merced counties. The ideal candidate will possess a strong foundation in community-based care, behavioral health, and social determinants of health. As a leader in our organization, you will champion health equity, foster interdisciplinary collaboration, and uphold the highest standards of CalAIM
compliance and cultural competency.

Essential Duties & Responsibilities

  • Conduct in-depth initial and ongoing comprehensive assessments to identify the complex clinical, behavioral health, and social needs of vulnerable member populations.
  • Develop, implement, and dynamically update individualized care management plans that align strictly with CalAIM Enhanced Care Management standards.
  • Act as the primary point of contact to coordinate seamlessly across primary care providers, behavioral health specialists, county services, and community support systems.
  • Facilitate smooth transitions of care (e.g., hospital discharge to home) and empower members with education for sustainable self-management.
  • Ensure strict operational adherence to HIPAA, CalAIM/ECM guidelines, and Department of Health Care Services (DHCS) reporting requirements.
  • Travel locally to engage with members directly in their homes, at appointments, or in community settings across the four-county service area.
Qualifications & Requirements

  • Verification of identity and legal authorization to work in the United States.
  • Successful completion of a comprehensive background check, including clearance for engagement with vulnerable populations.
  • Valid California Driver's License and proof of automotive insurance meeting state minimums.
  • Provision of a valid Tuberculosis (TB) screening result (reimbursable per company policy).
  • Language Skills: Bilingual proficiency is highly preferred and qualifies for additional compensation.
  • Certifications: Basic Life Support (BLS) Certification and other specialized ECM credentials are required, but training and certification can be completed with and provided by HATS upon hire.
Physical Requirements & Working Conditions

  • Hybrid Nature: Requires a mix of remote administrative work and frequent travel for in-person home visits, community meetings, and team gatherings.
  • Travel: Must have access to a reliable vehicle and be willing to travel within the assigned territory. (Mileage reimbursement provided per IRS standards).
  • Physical Demands: Ability to sit/stand for extended periods, operate a computer, and occasionally lift up to 15 lbs.
