We’re looking for a skilled Physical Therapist who thrives in new environments and enjoys bringing clinical impact to communities across sunny California. This unique internal travel role offers guaranteed 40 hours per week, 13-week assignments, and the chance to explore new communities through our network of Bridge Home Health locations. JOB DESCRIPTION SUMMARY

The Traveling physical therapist contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.

  • Assignment Duration: Standard travel assignments are 13 weeks in duration unless otherwise agreed upon in advance.

  • Weekly Hours: Full-time schedule of 40 hours per week.

  • Starting Location: Santa Cruz

  • Work Schedule: Schedule will vary based on assignment location. Weekend availability may be required depending on patient needs.

  • Employment Status: This position is direct of Bridge Home Health & Wellness. Upon completion of an assignment, you may be reassigned to a new contract within the State of California as determined by organizational needs.

  • Attendance & Reliability: Must adhere to company attendance and reliability policies, which are essential for delivering high-quality and consistent patient care.

  • Training & Team Support: Demonstrates a willingness to assist in onboarding and training new employees as requested and serves as a thought leader by sharing knowledge and best practices with the team.

 

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

  1. Provides physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act. This may include, but will not be limited to:

    1. Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care.

    2. Evaluates home environment.

    3. Assesses muscle strength, mobility, gait, ROM—potential for rehab.

    4. Directing physical therapy treatment.

    5. Instructing patients and families/caregivers in the use and care of therapeutic appliances.

    6. Determining priority needs for physical therapy.

    7. Reporting to physician patient’s reaction to treatment or changes in condition.

  2. Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.

  3. May train patients in the use of prosthetic devices.

  4. Identifies patient and family/caregiver needs for other home health services and refers as necessary.

  5. Prepares and submits clinical and progress summaries based on the attainment of goals.

  6. Participates in discharge planning for patients.

  7. Prepares and submits a clinical progress summary based on the attainment of goals as directed by Organization policy.

  8. Provides physical therapy consultation to home families/caregivers when indicated.

  9. Provides in-service education programs for nursing organization personnel as needed.

  10. Participates in peer consultation process.

  11. Supervises physical therapy assistants according to organization policy and state regulations.

  12. When therapy is the only skilled service, instructs, supervises, and evaluates home health aide care every two (2) weeks.

  13. Must adhere to the company attendance and reliability guidelines, which are an essential function of the job to ensure quality and consistent patient care.

POSITION QUALIFICATION

  1. Possesses a post baccalaureate degree in physical therapy (Doctor of Physical Therapy) approved by the Commission on Accreditation in Physical Therapy Education (CAPTE).

  2. For physical therapists licensed prior to the change in entry-level requirements (January 2016), may possess a baccalaureate or master’s in physical therapy from a program approved by CAPT. Licensed to practice as physical therapist by the Physical Therapy Board of California.

  3. Demonstrates good verbal and written communication and organization skills.

  4. Possesses and maintains current CPR certification.

  5. Possesses and maintains valid and current Auto Insurance.

  6. Must possess a valid Driver’s License with automobile that is in good working order and insured in accordance with the organization requirements.

  7. Must reside 55 miles or more from work location

  8. Maintains current and valid Driver’s License and Automobile Insurance coverage at all times.

  9. Must have a reliable form of transportation.

PHYSICAL REQUIREMENTS

  1. The ability to sit occasionally for 1-3 hours.

  2. The ability to frequently stand for 3-6 hours.

  3. The ability to occasionally drive/ operate foot control for 1-3 hours.

  4. The ability to walk occasionally on level surface for 1-3 hours.

  5. The ability to walk intermittently on unlevel surface for less than 1 hour.

  6. The ability to intermittently bend or climb for less than 1 hour.

  7. The ability to occasionally twist, kneel, crouch and crawl for 1-3 hours.

  8. The ability to lift 50 pounds intermittently in less than 1 hour.

  9. The ability to carry up to 50 pounds intermittently in less than 1 hour.

  10. The ability to push up to 50 pounds intermittently in less than 1 hour.

  11. The ability to pull up to 50 pounds intermittently in less than 1 hour.
