Build strong, compassionate relationships with residents and provide person-centered care Support residents with activities of daily living, including personal care needs Administer medications and monitor residents’ health and well-being Observe, assess, and communicate changes in residents’ conditions to the care team Collaborate with interdisciplinary team members to ensure high-quality care Maintain accurate documentation and follow care plans Uphold a safe, respectful, and welcoming environment for residents and staff Participate in additional duties as assigned to support resident care and team operations  Qualifications Active RN license in the State of Michigan (required) Long-term care experience (preferred) Subacute rehabilitation experience (preferred) Current CPR certification Ability to work both independently and as part of a team Strong communication skills and ability to relay resident condition updates effectively Compassionate, patient-centered approach with excellent bedside manner Reliable, professional, and committed to providing high-quality care
