These positions are servicing Washtenaw County. Job Duties: Assist clients with activities of daily living, including bathing, grooming, dressing, toileting, and personal hygiene. Assist with transfers, ambulation, positioning, and range-of-motion exercises. Follow individualized care plans and promote independence. Prepare meals and provide assistance with feeding and hydration as needed. Provide medication reminders as permitted by agency policy and state regulations. Provide companionship and encourage social, recreational, and therapeutic activities. Assist with light housekeeping, laundry, errands, grocery shopping, and other authorized tasks. Provide transportation and escort services to appointments and community activities when assigned. Support clients living with dementia using the Best Friends Approach to person-centered care. Observe and report changes in a client's physical, emotional, or cognitive condition. Maintain a clean, safe, and sanitary environment while following infection control procedures. Complete required documentation and communicate important information to the care team. Sign-On Bonuses: $2,000 for Full-time Positions working 0.8 - 1.0 FTE. $1,000 for Part-time Positions working 0.4 - 0.7 FTE. Part-time eligibility contingent on a commitment to work greater than 32 hours per pay period. QualificationsRequirements: Must be 18 years of age or older. Must possess a valid driver's license, maintain reliable transportation, and be able to provide transportation services to clients when required. Current CNA certification in good standing with the State of Michigan. High school diploma or GED required. One year of senior care, memory care, dementia care, previous HHA, or private duty healthcare experience preferred. Basic computer and documentation skills. Strong communication and interpersonal skills. Compassionate, patient, dependable, and positive attitude. Ability to work independently and follow care plans and safety procedures. Ability to stand, walk, bend, stoop, push, pull, and assist clients with mobility and transfers.
