One of LSC’s senior living communities, Trinity Oaks Health & Rehab, is seeking a full-time MDS Nurse. As an MDS Nurse at Trinity Oaks, you will work collaboratively to develop, organize, and monitor individualized care plans for residents from admission through discharge.
