This rewarding role offers the opportunity to directly impact the lives of individuals with developmental disabilities while building a fulfilling career in human services.   Locations We are hiring for all office locations: Stockton Modesto Tracy San Andreas (serving Tuolumne, Amador, and Calaveras counties) Note for San Andreas office: Your caseload location (Tuolumne, Amador, or Calaveras) will be based on where you reside.   What You'll Do: Champion for Others: Advocate for the rights and needs of individuals with developmental disabilities Connect People to Resources: Coordinate essential services that enhance quality of life Build Meaningful Relationships: Work directly with consumers, families, and community partners Key Duties for the Service Coordinator Position: Case Management & Planning Assess, coordinate, and implement individual program plans through team processes Schedule and facilitate various reviews and team meetings Documentation & Reporting Complete comprehensive case documentation including program plans, evaluations, progress reports Prepare service authorizations, court reports, and Medicaid documentation Advocacy & Support Provide advocacy, education, and crisis intervention for clients and families Serve as liaison between clients and various service providers Data Collection & Analysis Collect and report client statistical data for various surveys and assessments Professional Collaboration Participate in internal/external committees for education and agency improvement Maintain positive communication with partner organizations Emergency Response Serve as Officer of the Day and participate in After Hours Response System as assigned Areas of Support This position may support individuals in the Children or Transition/Adults programs, depending on assignment and organizational needs.   Why You'll Love Working With Us: Diverse Work Environment: Split your time between our welcoming office and out in the community, hybrid schedules available Never a Dull Moment: Each day brings new challenges and opportunities to problem-solve Professional Growth: Ongoing training to expand your knowledge and skills in developmental services   Join a Team That Makes a Difference!   As a Service Coordinator, you'll be the vital link between the individuals we serve and the resources they need to thrive.
