Under general administrative direction, manages the non-clinical care of patients with cancer DRGs across the oncology serviceline inÂ
conjunction with the patient’s multidisciplinary care team. Collaborates with the patient navigator in coordinating referrals fromÂ
physicians and health care facilities for patients; providing a significant level of patient education related to their illness and plannedÂ
treatment. Directs patients with cancer DRGs through the complex care continuum and assuring patients receive care that consistentlyÂ
follows evidence-based standards and is in accordance with regulatory standards as defined by the regulatory agencies. Identifies andÂ
interacts with community resources and develops relationships with service providers to enhance the patient experience and eliminateÂ
barriers to successful completion of care. Develops patient educational resources and support groups as part of the Whole Life WellnessÂ
program for all BHS tumor site programs. Manages national accreditation program standard compliance by ensuring staffÂ
accountability. Facilitates administrative activities in support of the service line staff and goals. Completes established competencies forÂ
the position within designated introductory period. Other related duties as assigned.
