Nexus Neurorecovery Center is a female adolescent residential treatment center located in the heart of San Antonio. Our post-acute, community-like setting supports residents navigating mental and behavioral health challenges, along with co-occurring medical needs.

Position Summary

The Registered Nurse (RN) provides direct, high-quality nursing care to adolescent residents with medical and behavioral health needs. Working overnight as part of a multidisciplinary treatment team, the RN delivers holistic, patient-centered care in accordance with the Texas Board of Nursing (BON) Nurse Practice Act, regulatory requirements, and Nexus Health Systems policies.

This role requires strong clinical judgment, critical thinking, patient advocacy, and the ability to support residents in a safe, structured, and therapeutic residential environment.

Job-Specific Responsibilities

Service

  • Supports and communicates the Mission, Vision, and Values of Nexus Health Systems

  • Upholds standards of conduct, ethical behavior, and corporate compliance

  • Maintains confidentiality and protects all PHI in compliance with HIPAA

  • Collaborates effectively with colleagues and interdisciplinary teams to ensure seamless care delivery

  • Demonstrates professionalism, integrity, and respect in all patient and staff interactions

Clinical Excellence

  • Provides nursing care to adolescent patients with medical and behavioral health diagnoses using evidence-based practices

  • Administers medications, treatments, and interventions per physician orders and nursing standards

  • Monitors patient status and identifies changes requiring intervention or escalation

  • Collaborates with physicians, therapists, social workers, and behavioral health professionals to implement individualized care plans

  • Provides crisis intervention and utilizes de-escalation techniques when appropriate

  • Educates residents and families on medications, health conditions, and coping strategies

  • Coordinates care for residents with co-occurring medical and behavioral health needs

  • Ensures adherence to safety protocols, infection control standards, and documentation requirements

  • Accurately documents patient care in accordance with regulatory and facility standards

Patient Experience & Advocacy

  • Advocates for residents to ensure individualized, compassionate, and comprehensive care

  • Promotes resident and family engagement in care planning and decision-making

  • Communicates proactively with families and caregivers, providing updates and addressing concerns

  • Demonstrates cultural sensitivity and respect for diverse patient populations

Quality Assurance & Compliance

  • Ensures compliance with all healthcare regulations, policies, and nursing standards

  • Promotes a culture of patient safety and identifies opportunities for improvement

  • Participates in quality improvement initiatives and best-practice implementation

  • Adheres to the Texas Nurse Practice Act and BON standards at all times

Professional Growth & Education

  • Completes all required annual education and competency validations

  • Maintains current knowledge of nursing standards, behavioral health trends, and best practices

  • Participates in continuing education and professional development activities

  • Engages in self-assessment, peer review, and staff education opportunities

Resource Stewardship

  • Uses medical supplies, medications, and equipment responsibly and efficiently

  • Supports accurate documentation for billing and regulatory compliance

  • Collaborates with the care team to deliver cost-effective, high-quality care

  • Performs other duties as assigned

Position Qualifications

Education

  • Associate Degree in Nursing (ADN) from an accredited nursing program required

  • Bachelor of Science in Nursing (BSN) preferred

Experience

  • Minimum of 2 years experience in an inpatient hospital or rehabilitation setting preferred

  • 1–2 years of pediatric nursing experience preferred

  • Experience with behavioral health and/or medically complex patients strongly preferred

Skills

  • Strong clinical assessment and critical-thinking skills

  • Ability to manage acute medical issues and behavioral health situations

  • Effective communication with residents, families, and interdisciplinary teams

  • Proficient in de-escalation techniques and behavioral health support

  • Strong organizational skills and ability to prioritize in a dynamic environment

  • Competent in EHR systems and clinical documentation

Licensure & Certifications

  • Current Registered Nurse (RN) license in the State of Texas

  • BLS (AHA or Red Cross) – required

  • ACLS – required per unit standards

  • PALS – required per unit standards

  • De-escalation training within 30 days of hire

  • All certifications must remain current and in good standing
