Position Summary


We are seeking a dependable and compassionate Certified Nursing Assistant (CNA) to provide personal care and supportive services to patients in their homes. The CNA works under the supervision of a Registered Nurse and follows the patient's established plan of care.

This position is ideal for a CNA who enjoys working independently in the community while being part of a supportive home health team.


Responsibilities

  • Provide personal care as directed by the patient's plan of care, including bathing, grooming, dressing, and hygiene assistance.
  • Assist patients with mobility, transfers, positioning, and ambulation as appropriate.
  • Assist with activities of daily living (ADLs).
  • Observe and report changes in the patient's condition to the supervising nurse.
  • Measure and document vital signs when assigned.
  • Maintain accurate and timely documentation of all visits and services provided.
  • Follow infection-control, safety, and patient-rights requirements.
  • Maintain patient confidentiality and comply with HIPAA requirements.
  • Communicate professionally with patients, families, nurses, and other members of the care team.
  • Travel to patient homes within the assigned service area.


Qualifications

  • Current Texas Certified Nurse Aide (CNA) certification in good standing.
  • Previous home health, hospice, long-term care, or similar patient-care experience preferred.
  • Current CPR/BLS certification preferred.
  • Valid driver's license and reliable transportation.
  • Current automobile insurance.
  • Ability to travel within assigned service areas.
  • Ability to work independently and manage an assigned patient schedule.
  • Strong communication and documentation skills.
  • Must successfully complete required background and compliance screenings.
