JOB DESCRIPTION

We are seeking a compassionate and experienced Hospice Admission Registered Nurse (RN) to conduct comprehensive patient assessments, develop individualized care plans, and coordinate hospice services for patients in their homes. The Admissions RN plays a critical role in ensuring a smooth transition into hospice care while providing education, support, and guidance to patients and families.

Key Responsibilities

  • Complete comprehensive hospice admission assessments, including physical, psychosocial, and medical history evaluations.

  • Develop and initiate individualized hospice care plans based on patient needs and goals.

  • Provide skilled nursing care, symptom management, and patient education.

  • Collaborate with physicians, interdisciplinary teams, caregivers, and community resources to coordinate care.

  • Educate patients and families on disease progression, medications, comfort care, and self-care techniques.

  • Monitor and document patient status, interventions, and outcomes in a timely and accurate manner.

  • Communicate changes in condition and obtain physician orders as needed.

Qualifications

  • Current and unrestricted California Registered Nurse (RN) License.

  • Minimum of 2 years of RN experience; hospice, home health, palliative care, or public health experience preferred.

  • Strong clinical assessment, care planning, and communication skills.

  • Current CPR/BLS Certification.

  • Valid driver’s license, reliable transportation, and current auto insurance.

  • Ability to travel to patient homes and perform physical job duties, including standing, bending, lifting, and transferring patients as needed.
