As a Home Health Care Specialist, you will:  

• Build and maintain strong referral relationships with healthcare providers and community partners.
• Generate new referrals and manage existing accounts.
• Develop relationships with RN case managers, discharge planners, physicians, and other healthcare professionals.
• Collaborate with clinical and operational teams to support patient care and ensure a smooth referral process.
• Obtain face-to-face physician documentation as required.
• Attend sales meetings and interdisciplinary team meetings.
• Represent Lorian Health professionally within the local healthcare community.

Required Qualifications

• Bachelor’s degree in a related field or five (5) years of previous sales or business development experience in home health or a related healthcare industry.
• Minimum of one (1) year of recent experience within the last three (3) years in a Home Health or Hospice setting.
• Working knowledge of Microsoft Office, including Word, Excel, and PowerPoint, as well as computerized systems in general.
• Strong knowledge of the local medical community preferred.
• Strong leadership, organizational, interpersonal, and communication skills, both written and verbal.
• Ability to communicate and present comfortably in front of small or large groups.
• Strong relationship-building and customer service skills.

Pay Range: $90,000-$120,000 commensurate with experience.
