This is a Part Time Temporary position in our Homecare department with an anticpated duration of 6 months. Typical schedule is Monday - Friday, 8:00AM - 4:30PM.Â
Job Summary: Under the direction and supervision of the In-Home Services CNA Supervisor, the Healthcare Companion and Delivery Assistants responsible for providing participants with 1-1 assistance with activities of daily living, transfers, ambulation, and ensuring safety during external medical appointments.
Duties and Responsibilities:
• Consults each day with the In-Home Services CNA Supervisor to be updated on St. Paul’s PACE participant’s status, changes in care plan and to confirm scheduled visits.
• Independently follow the assigned schedule, ensuring timely arrival at all scheduled visits.
• Report cancellations, unusual incidents, and any other pertinent information to the appropriate staff members in real time.
• Document services provided in participant’s electronic medical record.Â
• Assist with coordination and scheduling of medical companion services as needed.Â
• Provide any needed assistance in the home to prepare for medical appointments i.e. bathing, grooming, meal prep, etc.Â
• Personal ADL assistance (toileting, hygiene, dressing) as needed.
• Assists participants with transfers and ambulation
• Provide supervision throughout medical appointments as required to ensure participantÂ
safety
• Other duties as assigned
Qualifications and Requirements:
Education:
- High School diploma or equivalent required. If CNA, completion of formal CNA training program.
- Successful completion of First Aid and CPR training in person with the American Heart Association.
- For CNAs, current California CNA certification.
-  Valid driver’s credentials and reliable transportation is requiredÂ
Experience: Minimum of one year’s experience working with the frail or elderly
