Patient Care Coordinator (LVN) — Aligned Hospice

About the Role

Aligned Hospice is seeking a dedicated and detail-oriented Patient Care Coordinator to join our interdisciplinary team. This role is central to ensuring seamless communication and coordination between patients, families, and our clinical team. The ideal candidate is highly organized, communicates effectively under pressure, and thrives in a fast-paced healthcare environment.

Key Responsibilities

• Triage incoming phone calls Monday through Friday, ensuring timely and appropriate response to patient, family, and staff needs
• Coordinate patient visit scheduling and needs among RNs, CHHAs, and Medical Directors
• Assist in preparing documentation and materials for Interdisciplinary Group (IDG) meetings
• Manage medical supply deliveries and coordinate timely medication refills
• Complete and process clinical and administrative forms accurately and efficiently
• Fax and transmit documentation to partnering communities and facilities
• Manage and maintain the face-to-face visit schedule for Medical Directors and Nurse Practitioners
• Provide occasional PRN nursing visits to support staffing gaps as needed

Qualifications

• Active LVN license (required)
• Hospice experience strongly preferred
• Prior experience working within an interdisciplinary healthcare team preferred
• Excellent verbal and written communication skills
• Strong organizational skills and ability to multitask in a dynamic environment
• Comfortable managing a high volume of phone calls and coordinating across multiple stakeholders

Work Environment

This position is primarily office-based, with occasional field visits required to support PRN nursing coverage during staffing shortages.

Schedule

Full-time, Monday through Friday 8:00 AM to 5:00 PM
