Lead Peer Recovery Practitioner - Intensive Case Management

Program Description:

Journey to Wellness is an Intensive Case Management program serving adult clients in San Diego County. NAMI San Diego & Imperial Counties (National Alliance on Mental Illness) is sub-contracted with UPAC (Union of Pan Asian Communities) for this program.

Primary Responsibilities/Function:

Under the supervision of the Program Manager, the team lead provides treatment and relevant services to program clients in accordance with Intensive Case Management (ICM), Full Service Partnership (FSP) fidelity.

Duties and Responsibilities:

• Conduct designated initial and ongoing screening, assessment and diagnosis, utilizing relevant tools such as ASAM and LOCUS.
• Develop problem lists and effective treatment strategies to identified client needs.
• Triage and assess clients, provide case management, family education/support, provide individual and group therapy.
• Provide education, crisis intervention, case management and supportive services to clients and families.
• Review and complete diagnosis interpretation for assessments, ensure problem lists are updated as clinically relevant.
• Provide linkages and referral services to clients as needs are identified throughout treatment, including during assessment and discharge.
• Document all the services rendered, in a timely manner, and in compliance with CalAim and other applicable requirements and standards.
• Maintain, at minimum, a productivity rate of 50% monthly.
• Drive clients to and from services and appointments as needed in personal vehicles.
• Participate in assigned staff meetings and required trainings.
• Utilize evidence-based practices, including, but not limited to Motivational Interviewing, Relapse Prevention, and Trauma Informed Care model.
• Provide culturally competent services. Work well with transitional age youth, immigrants, criminally justice involved, refugees, and other diverse client groups.
• Conduct effective outreach to promote program services as assigned.
• Provide all other related services required by statement of work for program and other duties as assigned.
• All the duties and responsibilities shall be delivered in compliance with CalAIM, County of San Diego BHS OPOH, DHCS State Certification Standards and other relevant regulations.
• Other duties as assigned.

Accountability: Reports to NAMI Director.

• Spends approximately 70% of work time sitting and meeting with others or working at a desk and/or computer.

• Frequently is required to use hands to touch, handle or feel and reach with hands and arms.
• Occasionally lifts and/or moves up to 25 pounds.

Minimum Requirements:

• Must be registered with the Board of Behavioral Sciences (e.g. Associate Marriage and Family Therapist, Associate Professional Clinical Counselor or Associate Clinical Social Worker). (OR)
• Registered/Certified by one of the DHCS-approved certifying organizations for Substance Use Counselor or Peer Support Specialists (OR)
• Meet education and credentials for Community Health Worker or Mental Health Rehabilitation Specialist.
• Reliable and consistent transportation.
• Able to communicate effectively, verbally and in writing.
• Proficiency with computer software (Microsoft Office: Outlook, Word, Excel) and web navigation.
• Reliable and consistent attendance.
• Experience in working with clients with mental health and substance use diagnoses.
• Ability to establish and maintain supportive relationships with clients from diverse cultural backgrounds.
• Have knowledge and experience working with clients who have co-occurring disorders.
• Ability to work effectively with various referral sources including probation officers.
• Excellent organizational and time management skills as well as attention to detail; ability to manage multiple tasks, prioritize, and successfully bring them to fruition.
• Ability to function well as a member of an interdisciplinary team through effective communication skills and interpersonal skills.
• Ability to maintain strict confidentiality regarding sensitive or proprietary information or materials, abide by confidentiality laws such as HIPPA.
• Ability to work well under pressure.
• Excellent written and verbal communication skills.
• Must have flexibility to work some evening and weekend hours during scheduled client activities.
• Must have certified First Aid and CPR training (including infant CPR) and biennial
• Will not be debarred or excluded from participation in Federal programs by the General Services Administration and/or the Department of Health and Human Services of the Inspector General

Additional Preferred Education or Qualifications :

• Bilingual or bicultural in a County of San Diego threshold language.
• Knowledge and experience of Full-Service Partnership (ACT and/or ICM), SmartCare and Welligent data systems.

Salary ranges vary by experience, education and licenses/certifications:

Clinician (Associate or Licensed with BBS) $28.49 - $36.46

Certified Peer Support Specialist $25.00 - $32.00

Certified Community Health Worker $21.93 - $28.07

SUD Counselor (Registered or Certified) $27.76 - $35.54
