You’ll design and implement individualized therapy plans to help patients regain independence in their home environment while coordinating care with the broader rehabilitation team. Key Responsibilities: • Provide effective patient care using approved therapy techniques and procedures • Evaluate patients’ condition, function level, and treatment goals • Develop individualized treatment plans and update as needed • Supervise COTAs and HHAs per care plan requirements • Document treatment notes and progress in the patient’s chart daily • Assist patients and families with home exercise programs and adaptive equipment needs • Recommend and evaluate adaptive equipment for the home setting • Maintain compliance with HIPAA regulations and company policies • Coordinate total patient care with the healthcare team • Complete discharge summaries and obtain orders when needed • Submit patient documentation and visit records in a timely manner Qualifications Qualifications: • Licensed Occupational Therapist (OT) in the State of California • Minimum of 1 year of OT experience preferred • Experience with OASIS and WellSky (Kinnser) strongly preferred • Current physical (within the last 6 months) • Current TB test (within last 12 months) and CPR certification • Valid driver’s license and reliable transportation • Ability to attend a 2–3 day orientation at our San Diego office Apply Today!
