About The Opportunity
The Speech-Language Pathologist (SLP) evaluates, plans, and provides speech therapy services to adult patients in their homes in accordance with physician orders and the Plan of Care (POC).
Responsibilities
- Perform comprehensive speech, language, cognitive, and swallowing evaluations.
- Assess the home environment and recommend adaptive equipment as needed.
- Develop and update individualized Plans of Care in collaboration with the physician and care team.
- Provide skilled speech therapy interventions using evidence-based techniques.
- Monitor patient progress and modify treatment plans accordingly.
- Document visits accurately and timely in the electronic medical record.
- Communicate patient status to physicians, case managers, and interdisciplinary team members.
- Educate patients, families, and caregivers on therapy goals and home programs.
- Participate in case conferences, in-services, and team meetings.
- Follow infection control and HIPAA privacy standards.
Required Qualifications
- Active Speech-Language Pathologist license in the state of practice.
- Master’s or Doctorate degree in Speech-Language Pathology.
- Current BLS/CPR certification
- Valid driver’s license, insured vehicle, and ability to travel to patient homes
- Ability to work independently and manage schedule effectively
- Experience working with adult/geriatric populations
- Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.

This position is based on a Pay per visit compensation model.
Pay per visit ranges from $95 to $120 visit commensurate with experience and depending on type of visit.
