Summary:Ā Ā Ā Responsible for assisting the Short-Term Home Care Program and Housing Services Program, and performing office duties for the department.
Essential Functions:
ā¦Ā Ā Provides intake, assessment, scheduling and referral to seniors age 60 or over for their Short-term home care needs.
ā¦Ā Ā Develops a service plan for the consumer based on the assessment and establishes a schedule of services. Assigns Elder Care Worker(s) to perform services based on established protocol and follows up with consumers to assess the appropriateness and quality of services.
ā¦Ā Ā Provides information, referrals and counseling on available housing opportunities; assists clients in the application process by helping them with form completion and/or providing other needed assistance.
ā¦Ā Ā Maintains an effective referral system with relevant city departments, housing developers and building managers.
ā¦Ā Ā Provides presentations on housing-related issues; attends community meetings as assigned.
ā¦Ā Ā Maintains complete consumer files and completes all required data collection forms and reports.
ā¦Ā Ā Supports the Program Coordinator and assists with the day to day operations of the office by maintaining a clean and neat environment for clients and staffs. Replenishes office supplies as needed.
ā¦Ā Ā Prepares reports as required by funding sources.
ā¦Ā Ā Performs other duties as assigned.
Qualifications:
ā¦Ā Ā A high school graduate with relevant housing experience. Bachelorās degree preferred.
ā¦Ā Ā Experience in providing health, social or aging services in the community.Ā
ā¦Ā Ā Excellent verbal and written communication and presentation skills. Ā
ā¦Ā Ā Must be self-motivated, organized and able to multitask under minim supervision. Ā
ā¦Ā Ā Proficient in MS Office and the internet.Ā
ā¦Ā Ā Must be bilingual in English and Cantonese. Biliterate in English and Chinese a plus.
ā¦Ā Ā Must be able to travel via public transportation or personal vehicle for performance of job duties. If driving, must have and maintain a valid CA driver license, satisfactory driving record, and auto insurance coverage as specified in Self-Help's policy.
