About Us:

Santana Hills Healthcare Center is a 167-bed facility with 90 skilled nursing beds and 77 sub-acute beds, delivering compassionate, patient-centered care for rapid recovery in a central San Jose location.


We are seeking a compassionate, organized, and customer-focused Admissions Coordinator to join our Skilled Nursing Facility. In this role, you will serve as the first point of contact for residents, families, hospitals, and referral sources while coordinating seamless admissions and delivering an exceptional customer experience.

Responsibilities

  • Coordinate all aspects of the admissions process from referral through resident admission.
  • Develop and maintain strong relationships with hospital discharge planners, case managers, physicians, assisted living communities, and other referral sources.
  • Conduct facility tours for prospective residents and their families, highlighting services and answering questions.
  • Complete admission paperwork accurately and efficiently, obtaining all required signatures and documentation.
  • Enter resident information into the Electronic Medical Record (EMR) and other computer systems.
  • Collaborate with Nursing, Therapy, Environmental Services, Dietary, and other departments to ensure resident rooms are admission-ready.
  • Welcome and orient new residents and families to the facility, services, policies, and procedures.
  • Explain room rates, billing procedures, insurance coverage, Medicare, Medicaid, VA benefits, Social Security, and other financial resources.
  • Assist residents and families with healthcare decision-making documentation and advance directives when appropriate.
  • Provide exceptional customer service by greeting visitors, answering multi-line phones professionally, and responding promptly to inquiries.
  • Perform routine quality assurance rounds to ensure the facility is clean, welcoming, odor-free, and free of safety hazards.
  • Maintain an inviting reception area, including refreshments for visitors.
  • Communicate effectively with interdisciplinary team members to ensure a positive admission experience.
  • Maintain confidentiality and comply with HIPAA regulations.
  • Perform other duties as assigned.

Qualifications

  • High school diploma or GED required; Associate's or Bachelor's degree preferred.
  • Minimum of 1 year of admissions, healthcare, customer service, or marketing experience preferred.
  • Experience in a Skilled Nursing Facility, Long-Term Care, Rehabilitation, Hospital, or Assisted Living setting is strongly preferred.
  • Knowledge of Medicare, Medicaid, managed care, and insurance verification preferred.
  • Experience with EMR systems and Microsoft Office.
  • Excellent communication, interpersonal, and customer service skills.
  • Strong organizational skills with the ability to multitask in a fast-paced environment.
  • Professional appearance and positive attitude.
  • Ability to build relationships with referral partners and families.
  • Must be dependable, detail-oriented, and able to maintain confidentiality.
