The Specialist receives people who come for help, runs the standard coordinated entry assessment, prioritizes each person by need, and routes them to the right program with a clean handoff. The Specialist records every contact in CaseWorthy and keeps the front-door file accurate and audit-ready.  This is a front-line intake role. It is not a clinical role and not a long-term case management role. When a person needs clinical care or ongoing case management, the Specialist routes them to it.  The Coordinated Entry Specialist reports to the Community Triage Manager and works as part of the intake team. This is an onsite position.  The Specialist works across:  Stabilization programs, including Veterans Services  Economic Mobility programs, including Subsidized Employment and Employer Engagement  External referral partners, including county, VA, shelter, and reentry sources  Job Responsibilities  Screening and Assessment (40%)  Receive walk-ins, calls, and referrals as the first point of contact at the front door.  Run the standard coordinated entry assessment at intake, the same way every time.  Prioritize each person by need and urgency and recognize crisis situations.  Follow escalation steps when a person is in crisis or at risk.  Data and Documentation (30%)  Enter every contact in CaseWorthy accurately and on time.  Capture the eligibility documents each funder requires at the point of intake.  Keep the front-door record complete and audit-ready.  Routing and Referral (20%)  Route each person to the right program with a warm handoff across Stabilization and Economic Mobility.  Hand off to Economic Mobility when a person’s primary need is employment, so they do not restart intake.  Refer to outside agencies when Mission Services is not the right fit, with a clear reason and a real connection.  Follow-Up and Coordination (10%)  Follow up on pending handoffs so people do not fall through the gaps.  Keep a working view of the people in the intake pipeline.  Responsibilities may be added, deleted or changed at any time at the discretion of management, formally or informally either orally or in writing.  QualificationsRequired Qualifications/Experience  High school diploma or GED. An associate or bachelor’s degree is a plus, or an equivalent combination of education and experience.  1 or more years in intake, screening, front-desk, or client-services work in a human services or nonprofit setting.  Experience entering client data accurately in a case management or data system such as CaseWorthy or HMIS.  Ability to recognize urgent or crisis situations and follow escalation steps.  Basic Excel or Google Sheets.  Preferred Qualifications/Experience  Familiarity with coordinated entry or a standardized assessment tool. Santa Clara County uses the VI-SPDAT within its Clarity HMIS.  Direct experience in CaseWorthy or HMIS.  Experience serving veterans, reentry (AB109 and CFET), or workforce populations.  Bilingual in English and Spanish, with Vietnamese a plus.  Knowledge of the local Silicon Valley resource network.  Aptitudes/skills/temperament  Treats people in crisis with respect and moves at a steady pace, regardless of who walks in.  Accurate and consistent with data. Detail is the job.  Runs the standard the same way every time.  Comfortable at a high-volume, walk-in front door.  Clear, plain communication with clients and staff.  Key Performance Indicators  Success in this role will be measured by the ability to:  Complete standard screenings accurately and on time.  Route people to the right program quickly, with clean handoffs.  Keep CaseWorthy data complete and audit-ready.  Capture eligibility documents at intake.  Starting targets, sized to a team enrollment goal of about 1,200 a year. Baseline these in the first 90 days, then confirm with the Manager:  About 50 to 60 completed screenings a month.  90 percent of your contacts screened within 3 business days.  95 percent of your screenings routed within 1 business day.  95 percent or higher CaseWorthy data complete and on time.  95 percent or higher eligibility documents captured at intake.  Job Requirements  Onsite at Goodwill of Silicon Valley headquarters in San Jose. The front door is staffed in person.  Reliable transportation.  Computer literate. Knowledge of MS Office required.  Must pass a criminal background check.  Working and Physical Conditions  Location: Goodwill of Silicon Valley headquarters in San Jose. Regular in-person contact with the public, participants, partners, and community agencies.  Onsite role with daily front-door coverage.  Sedentary work, frequently sitting for extended periods in front of a computer monitor.  May occasionally require lifting objects up to 20 lbs.
