Location: San Jose Northeast and Southeast  Â
Schedule: Monday - Friday with on call weekends rotation  Â
As a Home Health Aide, you will:Â Â
-
Provide compassionate personal care and assistance with activities of daily living, including bathing, grooming, dressing, oral hygiene, toileting, skin care, and maintaining a clean and orderly environment. Â

Prepare and assist with meals and feeding as needed, while supporting patients’ nutritional and hydration needs. Â

Assist patients with ambulation, exercise, range-of-motion activities, and other care activities as directed by the Plan of Care and within the HHA’s training and scope. Â

Monitor and record vital signs and other patient observations as ordered and within established training requirements. Â

Assist patients with medication self-administration and other routine care needs as directed by the supervising nurse or therapist. Â

Observe patients for changes in condition and promptly report concerns or significant changes to the supervising nurse and case manager. Â

Maintain a safe, respectful, and supportive care environment while completing accurate documentation and following LORIAN’s care standards, procedures, and professional expectations.Â

Required QualificationsÂ
-
Must meet educational/experience requirements outlined in the laws of licensure states.Â
-
Certified/licensed and in good standing within state currently working in according to clinical type.Â
-
Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.  Â

Possesses and maintains current CPR certification.Â
-
Possess and maintains a valid driver’s license.Â
This position is based on a pay per visit compensation model. Pay per visit ranges from $27 to $35 visit commensurate with experience and depending on type of visit.Â
