Location:Â San Jose Northeast, Northwest, and Southeast
Schedule: Monday - Friday with on call weekends rotation  Â
As a Home Health LVN, you will:Â Â
Provide skilled nursing care in accordance with the RN-established Plan of Care (POC), physician orders, and applicable LVN scope of practice.Â
Monitor and assess patient condition, identify changes or concerns, and promptly communicate updates to the patient, family/caregiver, physician, and supervising nurse.Â
Deliver safe, high-quality home health services while assisting patients with activities of daily living and promoting independence and self-sufficiency.Â
Educate patients, families, and caregivers on medications, treatments, health management, and self-care.Â
Maintain accurate, timely, and thorough clinical documentation in accordance with Lorian policies, procedures, and regulatory requirements.Â
Coordinate patient care with the interdisciplinary team, identifying and communicating the need for additional services such as PT, OT, ST, MSW, CHHA, RN, or dietary support.Â
Participate in case conferences and ongoing care coordination while using supplies and equipment efficiently and maintaining a patient-centered approach to care.
Required Qualifications
- Certified/licensed to practice in the state of practice and in good standing.
- Graduate of an approved school of practical/vocational nursing.
- Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
- Possesses and maintains current CPR certification (BLS required)
- Possess and maintains a valid driver’s license.
This position is based on a pay per visit compensation model. Pay per visit ranges from $35 to $45 visit commensurate with experience and depending on type of visit.
