Position Details

  • Role: Occupational Therapist (OTR)

  • Location: White Blossom Care Center – San Jose, CA

  • Compensation: 50- $65.00/hour (DOE)

  • Schedule: Full-Time, Part-Time, or PRN

  • Program Type: Stable, In-House Therapy

Collaborative Team Culture

Join a large, multidisciplinary team of experienced clinicians and enthusiastic new grads in a fun, community-oriented setting featuring multiple open-air patios.

Key Responsibilities

  • Evaluation & Treatment: Perform comprehensive evaluations and deliver evidence-based occupational therapy interventions per physician orders

  • Clinical Leadership: Supervise and support Certified Occupational Therapy Assistants (COTAs) in compliance with California State Practice Acts

  • Interdisciplinary Collaboration: Participate in care conferences, utilization reviews, and rehab meetings to ensure coordinated, patient-centered care

  • Documentation Excellence: Complete timely and accurate evaluations, daily notes, progress notes, and recertifications in accordance with OT Board standards

  • DME & Patient Independence: Recommend, order, and train patients on durable medical equipment (DME) to promote safety and functional independence

  • Education & Discharge Planning: Provide caregiver training and education to families and nursing staff to support positive discharge outcomes

Qualifications
