Location:Ā San Jose Northeast and SoutheastĀ
Scheduled: Mon- Friday with on call weekends rotationĀ
Responsibilities
- Ā Assess patient needs and develop individualized care plans to ensure optimal outcomes
- Administer medications and treatments as prescribed by physicians and monitor patient responses
- Ā Collaborate with interdisciplinary teams to coordinate care and services for patients
- Ā Educate patients and families on disease management, treatment options, and self-care techniques
- Document patient information and care activities accurately and efficiently in electronic medical records.
Qualifications
- Ā Current licensure as a Registered Nurse in the state of California
- Minimum of 1 year of experience in home health or hospice
- Strong assessment and critical thinking skills to identify patient needs and prioritize care
- Excellent communication and interpersonal skills to work effectively with patients, families, and healthcare teams
- Ability to work in a fast-paced environment and adapt to changing priorities and situations
- Proficiency in electronic medical records and other healthcare technology systems

This position is based on a Pay per visit compensation model.
Pay per visit ranges from $100 to $170 visit commensurate with experience and depending on type of visit.
