Job DetailsJob Location: San Leandro, CA 94577Salary Range: $20.00 - $21.00 HourlyPOSITION SUMMARY The position supports adults with intellectual disabilities in building and maintaining successful employment in community-based work settings. This position provides individualized coaching, on-the-job training, workplace support, and follow-up services to help participants meet employer expectations and grow their independence at work. They also serves as a key contact between the participant, employer, supervisor, coworkers, and support team to encourage clear communication, positive relationships, and long-term job success. RESPONSIBILITIES Learn each participant’s strengths, needs, goals, and support plan by reviewing available records and maintaining regular contact through scheduled workplace visits. Provide hands-on job coaching, training, and support at community job sites based on each participant’s individual employment needs. Help participants develop and maintain appropriate work habits, job skills, communication skills, and workplace behaviors. Support participants in understanding job expectations, following workplace routines, completing assigned tasks, and maintaining positive relationships with supervisors and coworkers. Communicate regularly with employers, supervisors, coworkers, families, and other support team members as appropriate. Assist with the development and implementation of vocational training plans, follow-up support plans, and strategies that promote successful workplace adjustment. Monitor participant progress and provide verbal or written updates, evaluations, and recommendations when requested by the supervisor. Maintain accurate, timely, and complete documentation of services, including case notes, progress reports, quarterly reports, service hours, participant feedback, employer feedback, team contacts, and other required records. Protect the confidentiality of all participant records and information in accordance with program policies and applicable confidentiality laws. Attend team meetings, case reviews, and annual planning meetings for assigned participants when needed. Submit required employee paperwork, including timesheets, mileage, and reimbursement forms, accurately and on time. Provide services during evenings, weekends, or non-traditional work hours when participant schedules or program needs require it. Perform other related duties as assigned. One year of experience supporting adults with disabilities in a vocational, employment, or community-based setting preferred. Current CPR and First Aid certification required. Valid California driver’s license, good driving record, reliable transportation, and current auto insurance required. Strong verbal and written communication skills. Organized, detail-oriented, dependable, and able to manage time effectively. Ability to work independently and as part of a team. Strong problem-solving skills and ability to adjust to changing schedules and priorities. Ability to maintain professional relationships with participants, employers, coworkers, supervisors, and support team members. Able to complete duties efficiently, meet deadlines, and maintain accurate documentation. Ability to follow written, verbal, and schedule-based instructions.
