About PACE

Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality, affordable health and support services that preserve and protect their dignity, quality of life, and independence. The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly), affiliated with West Health (www.westhealth.org), is a non-profit that delivers medical, social, and behavioral services to the frail elderly to the North County of San Diego. This PACE center is entrepreneurial in nature and focuses on innovations and bringing new processes to the traditional PACE environment.

We're looking for passionate healthcare providers who want to be part of a team dedicated to enable seniors to live in their communities as independently as possible by providing compassionate, comprehensive health and supportive care.

One Team One Mission

Position Summary:

Under the supervision of Physical and Occupational Therapy, The Rehabilitation Aide will provide support to the Rehabilitation team GMWP.

Essential Functions:

  • Assist in scheduling, transporting, and transferring (in facility) participants as required for Physical and Occupational Therapy services
  • Assist participants in completing non-skilled/Maintenance/Group therapy under the supervision of Physical and/or Occupational Therapy
  • Document and communicate any scheduling conflicts with participants and appropriate staff
  • Distribute schedules and other needed information to the appropriate staff and/or participant
  • Assist the Physical Therapist and PTA with treatment (including, but not limited to, ambulation, ROM, hot/cold packs).
  • Assist the Occupational Therapist and COTA with treatment (including, but not limited to, ADLs, therapeutic exercise/activities)
  • Document patients' communication, engagement and progress consistently under supervision of Physical and Occupational Therapy.
  • Maintain general upkeep of Rehab Gym (sanitizing appropriate gym equipment, placing equipment in designated locations, performing linen laundry).
  • Remain updated on participant medical and physical information
  • Provide clerical support to Rehabilitation Staff as needed
  • Provide appropriate communication with other members of clinical staff (Including but not limited to (Providers, nurses, social workers, home care staff)
  • Attend staff meetings as directed

Education and Certification:

  • High school diploma or equivalent, required.
  • Certified Nursing Assistant preferred.
  • Current CPR and BLS certification, required.
  • Clean driving record.
  • Drivers license.

Skills and Experience:

  • Minimum of one (1) year of documented experience working with a frail or elderly population required.
  • One (1) year experience in a medical care setting as an Aide in an allied health field is desirable.
  • Bilingual English/Spanish, preferred.
  • Skilled in communicating with participants, their families, and other staff members; ability to establish a helping, trusting relationship with participants.
  • Ability to utilize appropriate safety measures when providing care.
  • Ability to work effectively and harmoniously as part of an interdisciplinary team, the elderly, providers of services, and peers.
  • Well organized, dependable, flexible, and resourceful.

Medical Clearance:

Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
