We’re growing and looking for an experienced Licensed Vocational Nurse (LVN) to join our Hospice Care team!

What You’ll Do

As a Hospice LVN, you will provide direct patient care under the supervision of a Registered Nurse (RN). You’ll follow the plan of care, deliver treatments, and collaborate with the interdisciplinary team to achieve positive patient outcomes.

Key Responsibilities:

  • Implement nursing care based on comprehensive assessment and plan of care
  • Document patient services accurately and timely
  • Educate patients and families on diagnosis and self-care
  • Coordinate hospice services and report needs to the care team
  • Manage pain and symptoms; evaluate patient response to medications
  • Use equipment and supplies effectively
  • Participate in quality improvement and professional development

What We’re Looking For

  • Active CA LVN License
  • Minimum 1 year of LVN experience (hospice experience preferred)
  • Current CPR certification
  • Valid driver’s license and insured, reliable vehicle
  • Ability to work independently and as part of a team
