- Pay Rate: Starting at $37.00/hour (Based on experience)
- Sign-On Bonus: $20,000 (2-year commitment required)
- Primary Practice Area: Outpatient with some Acute Care coverage
- Shift: Days (Based on department needs)
Position Requirements:

- Strong clinical assessment, treatment planning, and rehabilitation skills
- Ability to independently manage patient care plans and treatment programs
- Excellent communication, organizational, and problem-solving skills
- Ability to supervise Physical Therapist Assistants (PTAs), aides, and students
- Ability to work collaboratively with interdisciplinary healthcare teams

Education, License & Certification Requirements:
- Bachelor’s Degree in Physical Therapy – Required
- Active Physical Therapist (PT) License – Required
- BLS Certification – Highly Preferred (within one month of hire)
- Completion of required orientation and competency programs
Job Description:
- Evaluate patients through formal and informal assessments to determine functional abilities, including skeletal, neurological, respiratory, cardiovascular, and mobility status.
- Develop individualized treatment plans based on assessment findings, physician orders, and patient goals.
- Provide physical therapy interventions to restore, improve, and maintain optimal functional abilities.
- Educate patients, families, and caregivers regarding diagnoses, treatment plans, home exercise programs, safety strategies, and rehabilitation goals.
- Supervise Physical Therapist Assistants, Physical Therapy Aides, Home Health Aides, and students according to professional and regulatory standards.
- Recommend adaptive equipment and assist patients with obtaining appropriate supportive devices.
- Complete accurate and timely documentation required for patient care, billing, and regulatory compliance.
- Monitor patient progress and modify treatment plans based on changes in patient condition.
- Collaborate with physicians, nurses, case managers, and other healthcare professionals to coordinate effective patient care.
- Participate in interdisciplinary team meetings, quality improvement initiatives, and program development.
- Serve as a case manager for assigned patients when required.
- Promote physical therapy services through patient education and community outreach.
- Maintain patient confidentiality and follow all safety, infection control, and compliance standards.
- Perform additional duties as assigned.
