Cura Health is seeking a skilled and compassionate Registered Nurse Case Manager to join our team. In this essential role, you will coordinate and manage patient care plans, ensuring optimal health outcomes and seamless communication between patients, healthcare providers, and support services. The Registered Nurse Case Manager acts as a liaison to facilitate patient access to necessary medical and community resources while providing education and support throughout the care continuum. Key Responsibilities
  • Assess patients’ healthcare needs, develop individualized care plans, and coordinate services across multiple providers.
  • Serve as a primary point of contact for patients and families, providing education, support, and advocacy.
  • Collaborate with physicians, nursing staff, social workers, and community resources to ensure comprehensive patient care.
  • Monitor patient progress, evaluate outcomes, and revise care plans as necessary.
  • Facilitate discharge planning and promote smooth transitions across care settings.
  • Maintain accurate and thorough documentation of case management activities and patient interactions.
  • Ensure compliance with regulatory requirements and organizational policies.
  • Use clinical judgment to identify potential barriers to care and implement strategies to overcome them.
  • Valid and active Registered Nurse (RN) license.
  • Experience in case management, patient advocacy, or care coordination preferred.
  • Strong clinical knowledge and ability to assess patient needs comprehensively.
  • Excellent communication, negotiation, and interpersonal skills.
  • Ability to collaborate effectively with multidisciplinary teams.
  • Detail-oriented with strong organizational and documentation skills.
  • Familiarity with healthcare regulations, insurance processes, and community resources.
