The individual psycho-educational assessment service will include:
1. Â The administration of a battery of tests chosen in collaboration with the Special Education Department;
2. Â The scoring of all tests administered;
3. Â The interpretation of the test results;
4. Â The provision of a comprehensive written report describing the results of the assessment to the school within 30 days of the evaluation;
5. Â Documentation that parents and staff receive a copy of the individualized report;
6. Â Availability of the psychologist to participate in MDET/MDT/IEP meetings where the results of the testing are interpreted to parents/guardians and staff within 60 days of the evaluation;
7. Â Follow the Arizona Evaluation requirement format.
The individualized written psycho-educational report will include:
1. Â Basic student data as furnished by the referral packetÂ
2. Â Diagnostic statement categories listed in.....
4. Â Background information: educational status and history, health assessment and prior evaluation history;
5. Â Behavioral observations;
6. Â Assessments as requested on permission to evaluate with may include:
a. Â Intelligence
b. Â Academic achievement
c. Â Social behavior
d. Â Perceptual abilities
e. Â Emotional behavior
f. Â ADHD
g. Â Other assessments relevant to the performance of the individual child
7. Â Summary Report
8. Â Recommendations for placement, instruction, social, medical, etc.
