SUMMARY OF RESPONSIBILITIES:
The Discharge Planner is responsible for coordinating the discharge process for all CSU clients, post discharge follow-up, maintaining cooperative relations with community agencies and service providers, and meeting with families to develop and implement appropriate discharge plans for after care

MINIMUM QUALIFICATIONS:
This position requires a Bachelor’s Degree in a Behavioral Healthcare field with a minimum of 2 years of mental health related experience.  Master’s Degree preferred.Â

 ESSENTIAL DUTIES/RESPONSIBILITES:Â
- Provides and documents timely and thoroughly treatment services, assessments, interventions, and aftercare plans.Â
- Facilitates the exploration of continuing care issues that include: returning home, post program placement, participation in self-help groups, partial hospitalization, and intensive out-patient programs.
- Arranges appropriate post discharge referrals
-  Engages with collateral contacts and providers regarding client’s discharge needsÂ
- Participates in treatment teams to provide collateral information for discharge planning
- Enters information into CFBHN’s SharePoint system as required.
- Computer Literacy
- Completes all training requirements in a timely mannerÂ
- Completes all other duties and special projects as assigned
