We are currently seeking a skilled and motivated Physical Therapist (PT) to join our in-house team on a PRN basis. As a PRN Physical Therapist at Anew Healthcare and Rehabilitation, you will play a crucial role in providing rehabilitation services to residents on an as-needed basis. You will assess residents' physical abilities, develop tailored treatment plans, and collaborate with the interdisciplinary team to promote optimal functional outcomes.

Responsibilities:

- Conduct comprehensive assessments of residents' physical conditions and functional abilities.

- Develop individualized treatment plans based on assessment findings.

- Provide hands-on physical therapy to residents, including therapeutic exercises and modalities.

- Collaborate with other healthcare professionals to integrate physical therapy into overall resident care  plans.

- Monitor and document residents' progress, adjusting treatment plans as necessary.

- Educate and involve family members in the rehabilitation process.

Qualifications

- Master's degree in Physical Therapy from an accredited program.

- State licensure as a Physical Therapist.

- Experience in a skilled nursing facility or geriatric setting is preferred.

- Strong interpersonal and communication skills.

- Ability to work independently and as part of a collaborative healthcare team.
