Schedule / Expected Work Hours:
  • Monday through Friday, 9:00a - 5:00p.Ā 
  • The position may require occasional weekend or evening visits to client homes and participation in a clinical on-call rotation (phone support only).
Job Summary
Manages, supervises, coordinates, evaluates, and develops client care teams to ensure quality care delivery within the scope of services of the Agency and all applicable
policies, standards, laws, and state Nurse Practice Act.

Reports To:
Director of Clinical Services
Provide Supervision to: All direct care staff

Responsibilities
  1. Conduct in-home client visits such as start-of-care assessments, supervisory visits, and reassessment visits.
  2. Be available during business hours and on-call as needed to support quality care delivery.
  3. Receive referrals, assess service needs, and assign appropriate clinicians to cases.
  4. Meet regularly with clinical staff to provide guidance, set goals, and evaluate progress.
  5. Attend clinical meetings to facilitate care coordination and participate in on-call rotation.
  6. Support staff, clients, and families by providing direction on healthcare resources and educating them on clinical policies.
  7. Oversee accuracy and timeliness of clinical documentation and perform quarterly chart reviews.
  8. Assist with staff hiring, orientation, training, and conducting annual performance evaluations.
Qualifications
  • Graduate of an accredited school of professional nursing. BSN preferred.Ā 
  • Current license as a Registered Nurse in the state(s) of practice with at least two (2) years of homecare experience and one (1) year of supervisory experience.Ā 
  • Excellent verbal and written communication skills and strong interpersonal skills.
  • Must be a licensed driver with an automobile insured in accordance with Agency and state requirements.
  • Current CPR certification.
