Position Summary: 

Responsible for providing mental health services to patients on the inpatient unit, including assessment, counseling, education, discharge planning, acting as a community resource liaison, offering assistance to family members when appropriate and following up with patients.  The delivery of quality services and positive interaction with our customers is critical to the completion of all the tasks within this job description. 

These duties will be performed within the guidelines and standards of this institution and in accordance with state and federal statues where applicable. A goal of this position is to ensure that all contacts and customers receive quality services. The staff must be willing to participate in the quality improvement process, including, but not limited to, identifying areas in which quality improvement process can be applied; collecting and organizing quality measurement data; and participating in quality teams.

Education, License & Cert: 

Bachelor’s degree in social work, psychology, or a related field required.  Masters level preferred.

Experience: 

At least two years of experience working directly with psychiatric patients in either an inpatient or outpatient setting required. CPI certification required within 1 year of hire.

Essential Functions: 

The delivery of quality services and positive interaction with our customers is critical to completion of all tasks within this job description.

1) Provides evaluations, assessments, crisis intervention and referral for all populations served by the department in a psychiatric in‐patient setting.  Ability to work with confidential information in a tactful and diplomatic manner.

2) Provides direct treatment to clients and their families, when appropriate, in the form of individual, group and/or family counseling.

3) Meets and initiates therapeutic interventions with psychiatric in‐patients, their families and significant others to promote understanding and cooperation with the treatment and discharge planning process.

4) Documents in the patient’s record the TCM assessment, intervention, and all relevant interactions with the patient, family/significant others and appropriate community organizations.

5) Maintains knowledge of community resources that may be used in treatment planning.  Keeps an accurate listing of these resources.

6) Utilizes appropriate resources for complex problems that results in meeting the needs of the client and/or family and assesses patient’s and family’s post discharge needs and arranges follow‐up care and referral procedure.

7) Participates in addressing ethical and legal concerns of the patient/family and significant other that impact the treatment of the patient on the unit or post‐discharge.

8) Maintains confidentially on all patient, family and fellow employee information.

9) Participates in quality improvement activities as defined by the Behavioral Health Department leadership that are relevant to the care of clients and families.  Seeks to improve client and family outcomes through application of educational concepts and skills.

10) Maintains required departmental hours of continuing education per year.

Other Duties: 

1. Assist with case management needs in the Partial Hospitalization Program as needed.

2. Facilitate therapeutic groups as needed.

3. Other duties as assigned.
