TheĀ RecuperativeĀ CareĀ ProgramĀ (RCC)Ā isĀ aĀ medicalĀ respiteĀ shelterĀ programĀ forĀ singleĀ adultsĀ whoĀ areĀ dischargedĀ fromĀ aĀ hospital,Ā emergencyĀ room,Ā orĀ otherĀ healthĀ facilityĀ andĀ areĀ experiencingĀ homelessnessĀ whileĀ followingĀ anĀ ongoingĀ care/treatmentĀ plan.Ā TheĀ RCCĀ isĀ hiringĀ forĀ aĀ CaseĀ ManagerĀ II,Ā whichĀ isĀ theĀ journeyĀ levelĀ classĀ andĀ isĀ expectedĀ toĀ workĀ withĀ aĀ minimumĀ amountĀ ofĀ supervision.Ā IncumbentsĀ areĀ requiredĀ toĀ useĀ assessment,Ā interviewing,Ā andĀ counselingĀ skillsĀ toĀ performĀ moderateĀ toĀ complexĀ caseworkĀ services.

Special Requirements

Typical Job Duties:

ā—ProvideĀ ongoingĀ caseĀ managementĀ includingĀ assessment,Ā referrals,Ā budgetĀ assistance,Ā housingĀ placement,Ā andĀ promotingĀ housingĀ stabilityĀ withĀ individualsĀ asĀ assigned.
ā—AcceptĀ eachĀ participantĀ forĀ whoĀ theyĀ areĀ andĀ whereĀ theyĀ are,Ā andĀ thenĀ provideĀ options,Ā information,Ā andĀ choicesĀ thatĀ allowĀ forĀ stimulatingĀ change.

ā—OrientĀ newĀ shelterĀ guestsĀ intoĀ theĀ program,Ā including outliningĀ shelterĀ policyĀ andĀ expectations,Ā touringĀ theĀ shelterĀ facilityĀ andĀ diningĀ hall,Ā etc.

ā—FacilitateĀ storageĀ ofĀ personalĀ items.

ā—SupportĀ theĀ ProgramĀ ManagerĀ inĀ enforcingĀ andĀ maintainingĀ campusĀ andĀ shelter policy.

ā—SupportĀ theĀ DirectĀ ServicesĀ TeamĀ inĀ facilitatingĀ dayĀ services,Ā shelterĀ programs,Ā andĀ additionalĀ services.
