What You'll Do As a Home Health RN Case Manager, you'll develop meaningful relationships with your patients while coordinating and providing high-quality skilled nursing care. Responsibilities include: Manage an assigned caseload within the dedicated Scottsdale territory Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments Perform comprehensive nursing assessments and develop individualized plans of care Provide skilled nursing care, medication management, wound care, and patient education Monitor patient progress and communicate changes with physicians and the care team Coordinate care with Physical Therapists, Occupational Therapists, Speech Therapists, Medical Social Workers, and Home Health Aides Complete timely and accurate documentation using our electronic medical record system Help reduce hospitalizations while improving patient independence and quality of life Why You'll Love Working Here At Sunrise, our nurses are supported—not overwhelmed. Qualifications Required Current Arizona Registered Nurse (RN) License (or Compact License eligible for Arizona) Graduate of an accredited nursing program Current CPR Certification Valid Arizona driver's license Reliable transportation and current auto insurance Preferred Home health experience OASIS documentation experience Case management experience Experienced RNs who are interested in transitioning into home health are encouraged to apply.
