JOB DESCRIPTION
POSITION: TRBHA CARE COORDINATOR (Extensive Travel Required)
CLASSIFICATION CODE: EXEMPT FULL TIME DEPARTMENT: CLINICAL
REPORTS TO: TRBHA PROGRAM MANAGERF
STATEMENT OF JOB: This Position provides care coordination services to clients enrolled or eligibleÂ
to be enrolled with White Mountain Apache Behavioral Health Services Tribal Regional BehavioralÂ
Health Authority. The TRBHA Care Coordinator provides a link between the client, client supportÂ
system, community, and other healthcare and social service professionals. The TRBHA CareÂ
Coordinator addresses any barriers to care and provides referrals for needs such as food, housing,Â
education, and mental health services. In addition, they conduct outreach to engage communityÂ
residents, assist residents with health system navigation, and to improve care coordination. ThisÂ
position will also function as an advocate for the client with other service providers whenÂ
appropriate to do so.
This position requires Extensive Travel throughout the week, and occasionally during the eveningsÂ
and weekends. Travel is required to provide excellent care to clients placed in differentÂ
facilities located throughout Arizona.
DUTIES AND RESPONSIBILITIES:
1. Manages a caseload of clients/potential clients who would benefit from behavioral health services.
2. Documents contact clients to assess needs, maintain required paperwork, and engage familyÂ
members. in accordance with Arizona Health Care Cost Containment System (AHCCCS) and ABHSÂ
Standards.
3. Assist in the management of the high needs/high costs staffing and data transfer with AHCCCS.
4. Assist and manages the distribution of notices of admit from AHCCCS.
5. Abide by all applicable policies, procedures, and program standards of ABHS, White MountainÂ
Apache Tribe, Indian Health Services, Arizona Health Care Cost Containment System (AHCCCS) and other pertinent regulatory authorities.
6. Conduct outreach, facilitate access to healthcare services and provide education and support inÂ
accordance with the Arizona Health Care Cost Containment System (AHCCCS) guidelines for careÂ
coordination. Ability to demonstrate a high degree of flexibility and competence in adjusting toÂ
clientele with varying degrees of traumatic experiences and complex psychological presentations.
7. Ability to address a variety of diverse spiritual, emotional, and social needs in multipleÂ
environments of an often-high-risk population, using a trauma-informed care approach whileÂ
maintaining a high degree of professionalism and cultural awareness.
8. Facilitates engagement of clients and their families in appropriate healthcare services.
9. Possess the ability to work with a wide range of clients including adults and families.
10. Collaborates with court/probation, tribal/community agencies, families, and other naturalÂ
support systems to ensure client’s needs are met. Develops a strong working relationship withÂ
community partners.
11. Provides care coordination services at the clinic, in the community, adult shelters,Â
residential facilities, hospital settings and in client’s homes to ensure client obtains allÂ
benefits and services necessary.
12. Participates in multi-disciplinary meetings to advocate for client and community to coordinateÂ
consistent and timely provision of services; attends and participates in pertinent AHCCCS and IHSÂ
meetings as assigned.
13. Ability to facilitate referral process to assure continuity of care.
14. Ability to maintain client records in accordance with HIPAA, AHCCCS, ABHS, Federal, State, Tribal, and local requirements.
15. Maintain ethical policies and guidelines as outlined in accepted State ethics laws, rules,Â
policies, and principles and as outlined in discipline-specific code of ethics.
16. Provide a positive role model to clients and staff through leadership, self-discipline, andÂ
genuine care for the treatment and recovery of clients. All clients and staff members must beÂ
treated with dignity and respect.
17. Participation in all required ABHS training.
18. Reports/attends work in timely manner. Must be reliable and communicate directly withÂ
supervisor in all matters of attendance.
19. Performs other duties as assigned by supervisor.
MINIMUM QUALIFICATIONS:
1. Â 21 years of age and possession of a high school diploma or high school equivalency diploma andÂ
18 credit hours of post-high school education in a field related to behavioral health completed noÂ
more than four years before the date the individual begins providing behavioral health services andÂ
two years of full time behavioral health work experience; or high school diploma or high schoolÂ
equivalency diploma and four years of full time behavioral health work experience.Â
Associates/Bachelor’s degree in mental health/human services or related field preferred; experienceÂ
collaborating with Native Americans preferred.
2. Excellent written and oral communication skills; ability to speak the Apache language preferred.
3. Demonstrate critical thinking, problem-solving and organizational and time management skillsÂ
with ability to work independently, with flexibility within prescribed deadlines.
4. Strong interpersonal skills with an ability to work effectively with a wide range of people,Â
teams, managers, supervisors.
5. MS Office proficiency and the ability to correctly operate a copier and fax machine.
6. Valid Arizona Driver’s License and reliable transportation.
7. Ability to pass all pre-employment testing, including, but not limited to, fingerprint clearanceÂ
and background check.
