Your Role As a Home Health Physical Therapist, you will: Perform comprehensive physical therapy evaluations in patients' homes Develop individualized plans of care based on each patient's functional needs, goals, and medical condition Provide skilled physical therapy interventions to improve mobility, strength, balance, endurance, gait, and overall independence Establish measurable goals and continually assess patient progress Modify treatment plans as clinically appropriate Educate patients and caregivers on home exercise programs, mobility techniques, fall prevention, and safety Assess the need for assistive devices, durable medical equipment, and home safety modifications Coordinate and oversee care provided by Physical Therapist Assistants when applicable Communicate patient progress and changes in condition with physicians and the interdisciplinary care team Complete timely and accurate clinical documentation in accordance with Medicare, regulatory, and agency requirements Collaborate with nurses, therapists, physicians, and other healthcare professionals to provide exceptional patient-centered care Qualifications Required Graduate of an accredited Physical Therapy program Current and unrestricted Arizona Physical Therapist license CPR Certification (or ability to obtain) Valid driver's license Reliable transportation and current auto insurance Strong clinical judgment, communication, and organizational skills Ability to work independently while collaborating effectively with an interdisciplinary team Home health experience is preferred but not required. New graduates are encouraged to apply!
