We are currently seeking an experienced Registered Nurse Assessment Coordinator-
(RNAC) to join our collaborative team to support regulatory compliance, resident centered care and survey readiness.Â

Excellent communication and interpersonal skills are required for this multifaceted position.
This role is responsible to conduct and coordinate the development and accurate completion of the resident assessment and care plan process in compliance with all federal, state and local guidelines which govern the facility. Strong knowledge of MDS, the RAI and care planning process, including the implementation of CAAs.
Coordinates scheduling of OBRA and Medicare/managed care assessments, entry/re-entry records, and care plan reviews in accordance with the most recent regulatory guidelines.
Scheduling of all Medicare Part A and/or Managed Care MDS assessments in accordance with applicable RAI and state guidelines
Inform residents, responsible parties and staff of Medicare/managed care coverage status through appropriate documentation in accordance with all federal, state and facility guidelines. Provides updates as necessary. Works with Community Health Choice (CHC) case managers for Managed Medicaid providers Participate in committees as directed.
Qualifications
- Associate or Bachelor Degree in Nursing preferred. Previous experience in completion of Resident Minimum Data Sets and Care planning preferred. Basic computer knowledge is essential.
- LICENSURE/CERTIFICATION: Current RN license is required
- Preferred certification: RAC-CTÂ
- Strong organizational and communication skills
