You will collaborate and communicate with the multidisciplinary care team—participating in patient rounds, discharge planning, and follow-up outreach—to ensure continuity of care. Responsibilities include making appropriate referrals to VMFH nutritional services, implementing nutrition care plans, and delegating tasks to care team members in accordance with departmental policies while providing feedback and escalating concerns as needed. Additionally, you will promote nutrition education across assigned areas, develop and present educational classes within FHS and the broader community, and may provide training to department staff to enhance knowledge of nutrition, modified diets, and effective meal delivery practices. You are responsible for completing all required documentation in a timely, accurate, and thorough manner, including nutrition care documentation and charting in accordance with VMFH and department-specific standards, and ensuring all applicable codes are correctly entered.

To be successful in this role, you will demonstrate strong clinical judgment and critical thinking, effective communication and collaboration skills, and the ability to manage multiple priorities while maintaining accuracy and attention to detail.
