Advanced Practice Provider (Primary Care)
Location: Seattle, WA and surrounding areas

Serene Healthcare Staffing is seeking an experienced Advanced Practice Provider - Nurse Practitioner (NP) or Physician Assistant (PA) for our client in Seattle to provide comprehensive primary care to our members. You will be an integral part of the Seattle clinical team, practice holistic primary care via telehealth, in-person and in-home visits, and help define operational and care delivery protocols for a growing regional presence.

Key Responsibilities

● Provide in-home, on-site, and virtual primary care services, including preventive care, chronic disease management, and acute care, in collaboration with our physicians.

● Develop individualized care plans tailored to patients’ needs.

● Communicate effectively with patients, families, and the care team to ensure seamless coordination of care.

● Maintain accurate and timely medical documentation.

● Participate in team meetings, case reviews, and quality improvement initiatives.

● Uphold Together Health’s commitment to providing compassionate, accessible, and personalized healthcare.

● Virtual visit coverage and support for Together Health’s clinical team in surrounding cities within the state of Washington.

● Proficient in documenting and engaging with patients within our EMR.

● Assistance with care coordination for patients including facilitating appointments and transfer of appropriate referral information.

● Ensure accurate and timely medical documentation within our EMR.


Requirements

Qualifications

● Active Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of Washington, with primary care experience

○ Multiple state licensed a plus

● Pediatric experience is preferred

● Minimum of 5 years of experience in primary care.

● Unrestricted driver’s license

● Strong clinical skills in preventive care, chronic disease management, and acute care.

● Exceptional communication and interpersonal skills.

● Ability to work effectively in a collaborative, team-oriented environment.

● Passionate about patient care and making healthcare more accessible.

● Internet access

● A computer or laptop (Mac or Windows)
