Job Summary

Supports census development, care coordination and care transitions for Prisma Health Hospice of the Foothills to advance the growth, affiliations and customer service objectives of the organization. Implements regional referral development and affiliation initiatives by broadening the base of referring physicians, hospitals, facilities and other referring providers. Works with hospice program/service leadership, Prisma Health physician liaisons, and Prisma Health marketing department to assure consistency of messages and activities in the advancement of the organization's growth and business objectives. Serves as liaison to the community and region by establishing relationships and activities to increase awareness and knowledge of hospice services.

Essential Functions

  • All team members are expected to be knowledgeable and compliant with Prisma Health's purpose:  Inspire health.  Serve with compassion.  Be the difference.

  • Promotes, maintains and documents communication between physicians and clinical staff regarding patient care and agency function. Confers with supervisors and attends team meetings.

  • Maintains a current knowledge of Social Work needs, Billing practices and levels of care, and overview of clinical requirements needed for admission.

  • Promotes and expedites communication between hospice clinical staff in the field and physicians and their medical staff regarding patient care and agency function. Coordinates patient transfer onto hospice services. Transmits patient notes as required for quality patient care. Promotes core Prisma Health services through on site visits and interactions with physicians, staff members and administration.

  • Develops strategies and identifies barriers to conversion of business and collaborates for resolution to promote growth.

  • Creates, maintains and develops educational content for our provider practices on the ways in which they can refer to our various services.

  • Creates and maintains appropriate physician content to help market and promote new services.

  • Attends and participates in public relations functions in assigned communities.

  • Supports the Hospice Foundation and marketing of such to the community.

  • Performs other duties as assigned.

Supervisory/Management Responsibilities

This is a non-management job that will report to a supervisor, manager, director, or executive.

Minimum Requirements

  • Education - Bachelor's degree in a related field of study.

  • Experience - One (1) year of experience in healthcare, business, marketing or sales. Clinical healthcare experience preferred.

In Lieu Of

In lieu of a bachelor's degree, will consider associate's degree in a related healthcare field.

Required Certifications, Registrations, Licenses

  • A valid driver’s license; an acceptable motor vehicle record as defined by the Acceptable Motor Vehicle Record (MVR) Chart; and proof of auto insurance.

  • Driving is required for this position using the employee’s personally owned vehicle. Employee must have reliable transportation and will be expected to adhere to the Prisma Health Driver Safety Policy and specific department driving policies. Employee must pass driver training.
