We are seeking a Adult Blended Case Manager (BCM) to join our team at our program in Delaware County, PA.

 

Earn $22.38/hr.! 

 

Position Details

The Adult Blended Case Manager is a community-based position that provides resource coordination, recovery services, and symptom management, using a strength-based case management model, for up to 30 adults living with mental illness. The BCM will also provide information and support to families and significant others of the individuals served. The BCM is also required to complete paperwork, correspondence, and chart documentation.  

 

This position requires travel to various schools, homes, and other community locations to provide services and support clients.

 

Mileage reimbursement is provided.

Qualifications

The ideal candidate will possess one of the following:

  1. A high school diploma plus a minimum of 12 college credit hours in Psychology, Sociology, Social Welfare, Gerontology, or other social science required, and two (2) years of behavioral health direct care experience required; or
  2. A bachelor’s degree with major course work in sociology, social work, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, healthcare administration, counseling, or education.

The ideal candidate will have both of the following:

  1. Must possess a valid driver's license, insurance, and have a clean driving record.
  2. Own a reliable vehicle.
