We are seeking a Substance Use Disorder Case Manager to join our team in  Delaware County, PA.

 

Earn $22.94/hr.! 

 

Position Details

This position is based in Sharon Hill, PA and serves individuals throughout Delaware County, Pennsylvania. To ensure high-quality, individualized support, caseloads are maintained at no more than 30 active clients.

Under the supervision of the Program Manager, the Substance Use Disorder Case Manager provides advocacy, care coordination, and resource linkage services for individuals experiencing substance use disorders, mental health conditions, and co-occurring challenges. The Case Manager plays a vital role in helping clients access community-based services, build stability, and achieve their recovery goals.

Key responsibilities include collaborating with and coordinating services through community partners and systems such as the Department of Public Welfare (DPW), Social Security Administration (SSA), mental health and drug and alcohol treatment providers, adult and juvenile probation offices, managed care organizations, and the Office of Behavioral Health. The Case Manager serves as a trusted advocate, ensuring clients receive the support, resources, and services needed to enhance their overall well-being and independence.
