JOB TITLE: Physical Therapist Assistant

ORGANIZATIONAL UNIT: CPN Health Services 

Department: Administration

SUPERVISOR: Registered Physical Therapist

FLSA: Exempt

SALARY RANGE: $30,000 - $80,000

ALL FULL TIME POSITIONS:  In addition to your pay, your salary also includes Medical, Dental and Vision Insurance. The Biweekly deduction from employee paycheck is $40.00 for Employee Only, $95.00 Employee plus Children, $120.00 Employee plus Spouse, and $140.00 Employee plus family.  

CPN Health Services personnel may be assigned to provide services to patients of all ages including but not limited to Indian Children and will require a criminal history background screening.

JOB SUMMARY:  Primarily responsible for patient care as set forth by the treatment plan developed by the licensed and registered Physical Therapist.  

ESSENTIAL FUNCTIONS:

· Implement the patient treatment plan established by the PT.

· Instruct and counsel patients regarding therapeutic procedures.

· Accurately document changes in the patient’s condition.

· Communicate effectively with the supervising PT regarding the patient’s condition and monitor the treatment plan.

· Maintain compliance by adhering to CPNHS Policy and Procedure

· Maintain CEU requirements dictated by the Oklahoma State Medical Board.

· Maintain a clean working environment.

ADDITIONAL DUTIES: 

· Perform other duties and projects as requested by management.

KNOWLEDGE, SKILLS AND ABILITIES:

· Display competency in skills learned in educational setting

· Display any site-specific competency required by CPNHS.

MINIMUM QUALIFICATIONS FOR CONSIDERATION:

· Graduate of an accredited PTA program

· Maintain a current license and be in good standing with the state licensing board. 

Physical Requirements:

· Sitting and standing for prolonged periods of time.

· Moderate stooping, kneeling, and reaching are required.

· Performing repetitive work at a computer.

· Moderate lifting will be required.

ENVIRONMENTAL CONDITIONS: 

· Health facility office environment.
