Job Title: Behaviorist 

FLSA Status: Exempt

Salary Range: See Pay Scale 

 

Job Summary: Works as part of a multidisciplinary primary care treatment team to support whole-person care for Senior TLC participants. Provides behavioral health and substance use screening, brief intervention, education, referral, and consultation services within primary care settings, and supports treatment planning for participants with behavioral, psychosocial, and chronic health needs.  Position reports directly to the Behavioral Health Lead.

 

Specifications

Education: Master’s Degree required.

Experience: Minimum two years’ experience in a public health setting. At least 1 years experience working with the frail elderly population. 

Number and Type of Employees Supervised (optional): None

Licensure, Registry or Certification Required: Licensed Clinical Social Worker (or equivalent) in good standing in North Carolina.

Special Training: Meet a standardized set of competencies for the specific position description established by Senior TLC, Inc. and approved by CMS before working independently. 

Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact

Ages of Patients Rendered Care: 

 Neonate/Infant Early Childhood Adolescent Adult Geriatric All Age Groups

Key Responsibilities: (*denotes an age-related skill or task)

· Maintain adherence to professional Code of Ethics.  

· Provide evidence-based assessment, intervention, consultation, and referral for behavioral and chronic health conditions.  

· Collaborate as an integral member of the interdisciplinary healthcare team to improve patient care and efficiency.  

· Works collaboratively as part of an interdisciplinary treatment team sharing relevant clinical information and developing specific, individualized treatment goals and interventions.   

· Develop, implement, and facilitate behavioral health, substance use, and chronic disease care plans using appropriate treatment modalities and community resources.  

· Support primary care providers with screening, identification, diagnosis, and management of behavioral health, substance use, and chronic disease concerns.  

· Participate in ongoing assessment and quality improvement efforts related to behavioral health integration.  

· Provide consultative services and training support to care team members as appropriate.  

· Utilizes the electronic medical record (EMR) to document participant encounters, track interventions, and ensure accurate and timely clinical documentation in accordance with organizational and regulatory standards.  

· Uses Microsoft Office applications, including Microsoft Word, to create and maintain reports, correspondence, and other required documentation to support participant care and departmental operations.  

· Engage in continuing education to maintain licensure and clinical competency.  

· Works with professionalism and respect to all participants, families, colleagues, supervisors, and visitors of Senior TLC.  

· Supports Senior TLC’s mission to encourage and support the quality of life of seniors wishing to continue living in the community; its vision to be the preferred provider of individualized care for seniors in the community; and its values of respect, integrity, accountability, compatible goals, and compassionate care.
