Refers patients to community resources to assist in recovery. Investigates child abuse cases and takes authorized protective action when necessary. Counsels patients to help them overcome dependencies, recover from illness, and adjust to life. Monitors, evaluates, and records client progress according to measurable goals described in treatment and care plan. Modifies client treatment plans to comply with any changes in status. Organizes support groups or counsels family members to help them understand, deal with, and support the patient.
