This role is perfect for a Physical Therapist who is passionate about helping residents improve mobility, regain independence, and enhance their quality of life in a supportive long-term care environment.

Position Responsibilities:

Perform physical therapy evaluations and create customized treatment plans
Deliver high-quality therapy services to short-term rehab and long-term care residents
Focus on improving strength, mobility, balance, and daily function
Collaborate closely with nursing, therapy, and interdisciplinary teams
Educate residents and families on treatment plans and progress
Complete timely and accurate documentation

Qualifications:

Active Physical Therapist license in KY
Skilled Nursing or geriatric therapy experience preferred
Strong clinical and communication skills
Positive, team-oriented attitude
Passion for resident-centered care
