POSITION SUMMARY:
The Lead Triage Registered Nurse (Lead Triage RN), under the direction of the Director of Nursing and the clinical care team, serves as an integral member of the nursing leadership teams. The Lead Triage RN participates in administrative and organizational leadership activities, assesses patient needs, prioritizes according to urgency, provides evidence-based nursing advice and initiates, or directs care to the appropriate resources over the phone and through the electronic medical record (EMR) for CCI’s patient population.Â
As a member of the health center’s integrated care team, the Lead Triage RN fulfills the following KEY FUNCTIONS AND RESPONSIBILITIES for the provision of patient-centered, coordinated patient care:
- Utilizes evidence-based resources to provide nursing triage services: assesses medical history, clinical need and urgency and determines necessary disposition via telephone or web consult. Â
- Provides patient education, medical advice and preventative behaviors that support the patient’s signs and symptoms.Â
- Leads the transition of care outreach program by ensuring follow up for patients following hospital discharge or emergency room visit.Â
- Serves as a lead in triage, transitions of care management and patient communication.Â
- Develops, enforces, and maintains policies, procedures, workflows, and clinical guidelines. Â
- Engages in CCI quality assurance and quality improvement activities.Â
- Supervises triage nursing team – including assigning duties, coaching, hiring, training, and corrective action.Â
- Attends and participates in meetings, committees, health fairs, events and trainings as assigned. Â
- Consistently seeks avenues for self-education to maintain compliance with evidence-based practice and advancements in triage nursing. Â
- Maintains the patient’s electronic medical record within expected timeframes, following CCI standards for charting.Â
- Provides quality customer service and communicates in a HIPAA compliant, culturally sensitive, and trauma-informed manner. Â
- Works within the policies and procedures of CCI Health Services Inc.Â
- Performs other duties responsibilities as assigned by the Director of Nursing or designee.Â
MINIMUM QUALIFICATIONS:
- Active RN license issued by the State of Maryland Board of Nursing or any state under the Nurse License Compact. Â
- Minimum of three (3) years of nursing experience, with acute careÂ
- Progressive leadership experience a plus. Â
- Current BLS/CPR certification required.Â
- Excellent written, verbal, telephone, and interpersonal communication skills with experience documenting in an Electronic Medical Record (EMR). Specific knowledge of eClinicalWorks (eCW) is a plus.Â
- Prior experience in a lead or supervisory capacity a plus.Â
- Written and oral fluency in English. Bilingual (English/Spanish) preferred.Â
