Hospice RN Case Manager
As a Hospice RN Case Manager with Holy Cross Home Care & Hospice, you’ll use advanced technology, strong clinical judgment, and a patient‑centered approach to deliver exceptional one‑on‑one care. You’ll assess, plan, implement, and support care that improves outcomes and promotes independence—while working in an environment where your expertise is valued and supported.

Minimum Qualifications
Graduate of an accredited or state‑approved practical nursing program
Active LPN license in the State of Ohio
Minimum of one (1) year of acute care experience (home care experience preferred)
Phlebotomy and wound care experience required
Successful completion of the NAPNES medication exam or equivalent prior to medication administration
Minimum Qualifications:
Graduate of an approved nursing education program
Active RN license in the State of Maryland
1+ years of clinical nursing experience (Hospice preferred)Â
Strong communication, assessment, and organizational skillsÂ
Compassionate, dependable, and mission-mindedÂ
Must have current Driver’s license and reliable transportation
